To my knowledge, my pet is fit and healthy for grooming. I have disclosed all known risks, dangers, and medical conditions associated with my pet and have disclosed any preexisting conditions to the team . I release the groomer, hospital, and all affiliated agents from any/all liability associated with worsening an existing medical condition that results from grooming.
My pet is current on the required vaccines: Rabies (feline and canine), Distemper (feline and canine), Leptospirosis (canine), and Bordetella (canine). CIV (Canine Influenza) is highly recommended.
I authorize Partners Animal Hospital to do whatever they deem necessary for my pet's safety, health, and well-being while under their care, including emergency professional veterinary treatment. In the rare event of a medical emergency during grooming, I consent to the doctors and team to stabilize and medically treat my pet while they contact me. I understand I am financially responsible for the cost of treatment. If I choose not to authorize treatment, my pet will be picked up immediately and must be cleared by a veterinarian for future grooming services at this facility.
Mat Removal Policy
If my pet has severely tangled and/or matted fur, there is a greater risk of injury, stress, and trauma. All precautions will be taken. I understand that the de-matting procedure increases the risk of injury to my pet. I will not hold the groomer, hospital, and/or all affiliated agents liable for injury or medical expenses resulting from my pet being de-matted. The de-matting process is time-consuming, and additional grooming charges may apply.
Sedation Policy
If it is determined that my pet will require sedation to be groomed at a time after check-in, the groomer will make every effort to contact me at the numbers provided. If I cannot be reached to give authorization to sedate, the grooming service will be rescheduled for a future time. If sedation is performed, additional costs will apply, including a physical exam and pre-anesthetic blood work. I understand that some risks always exist with sedation/anesthesia.
Senior Pets
I understand seniors may have a greater risk of injury or fatigue. If my senior pet becomes too stressed or uncomfortable, they will get breaks until they are comfortable enough to proceed with the groom. Due to this process, senior pets may take longer to groom and/or incur additional fees.
Aggressive or difficult-to-handle pets requiring assistance may be charged additional handling fees.
Appointment Policies
Cancellations should be made more than 24 hours in advance. Late cancellations and no-shows will be charged a fee of $45.
Dropping off more than one hour before the scheduled appointment or leaving the pet more than 2 hours after completion of the groom will be charged a $20 fee per pet.
A pick-up time is to be arranged at the time of check-in. I agree that either I or an authorized agent of mine will promptly pick up my pet and pay for all accrued charges at the agreed-upon date and time. I understand that if I fail to contact the hospital and they cannot reach me, my pet will be considered abandoned after five days. The hospital may handle this abandonment in the best interests of the pet and/or the hospital following the local laws, and I will be responsible for all related fees, including, but not limited to, all medical and/or grooming services performed, costs associated with keeping the animal, or collection fees.
Partners Animal Hospital groomers are happy to correct grooms within three days.
